Mobile sports physicals
A preparticipation evaluation done at your home or your team's location. In Texas, school athletics generally run on UIL forms, and the provider decides clearance.

What a preparticipation evaluation covers
A structured history and physical examination intended to identify conditions that would make sport unsafe — particularly cardiac conditions, prior concussion, and unresolved musculoskeletal injury.
It is a screening for safety, not a general health check and not a clearance guarantee. The provider decides clearance based on what they find.
Texas paperwork
Texas public school athletics operate under University Interscholastic League rules, and UIL publishes the preparticipation physical evaluation forms districts use. The history portion is completed by the student and a parent; the examination portion is completed and signed by the provider.
Confirm two things with your district before booking: which form version they require this school year, and whether they accept a provider signature from outside a named clinic. Club and private leagues frequently use different paperwork entirely, and a form accepted by one is not automatically accepted by the other.
What can lead to deferred or withheld clearance
A deferral is the screening working. It means something needs looking at before sport, not that anything is necessarily wrong.
- A murmur, an abnormal rhythm, or a history of fainting during exertion.
- A family history of sudden cardiac death at a young age.
- A recent concussion without documented resolution.
- An unresolved musculoskeletal injury.
- Blood pressure outside the expected range for the athlete's age.
Why timing matters
Leave time for a deferral. If something is flagged a week before tryouts, the athlete needs an appointment with a specialist, and specialist appointments are not same-week in August.
Four to six weeks before the season is the useful window. A physical completed in June leaves room; one completed the night before does not.
Questions to ask a provider
- Do you complete the current UIL form, and will you sign it at the visit?
- Has our district accepted your signature before?
- What happens if something is flagged — do you refer, and how quickly?
- Do you offer team pricing, and what does it include?
- What should the athlete bring, and does a parent need to be present?
Common questions
Is a sports physical the same as a school physical?
No. A sports physical is an athletic safety screening on athletic paperwork. A school physical relates to enrollment health requirements and usually travels with immunization records. Some visits can cover both; confirm before assuming.
Can a whole team be seen at once?
Many providers offer group sessions for teams. Ask about throughput, what space is needed, and how parental consent is collected for minors before arranging one.
