What this form does

You tell us the kind of service you are exploring, roughly when you would like to talk to someone, and the address where a visit would happen. We compare the ZIP code against what each participating provider has told us it covers and is currently accepting; the street address is passed on only if you are matched.

If there is a match, we pass your request to that provider and they contact you. If there is no match, we say so plainly and nothing is sent anywhere.

What we deliberately do not ask

Several fields you might expect are missing on purpose. The less we hold, the less there is to lose or misuse.

  • Symptoms, diagnoses, or any medical history. There is no free-text field for them anywhere in this form.
  • Your city. Your ZIP code already determines it, so asking twice would only be one more thing to type.
  • Your date of birth, insurance member number, or Social Security number.
  • Any payment information. We never take payment for care.

What happens after you submit

  1. We check coverage

    This happens immediately. You will see the result on the confirmation screen, including the provider's name if there is a match.

  2. Your request is sent, or it is not

    A matched request goes to that one provider. An unmatched request is kept so we can tell you if coverage changes, and is not sent to anyone.

  3. The provider contacts you

    Timing is theirs to decide, not ours. Submitting this form does not create a provider-patient relationship and does not reserve an appointment.

Your information

We record what you submitted, the exact consent wording you saw, when you accepted it, and where you were on the site when you did. That record exists so that if you ever ask what was shared and with whom, we can answer precisely.

You can ask us to delete our copy at any time. We cannot delete a copy a provider has already received, and we will tell you that rather than implying otherwise.

Common questions

Does this cost anything?

Submitting a request is free and does not obligate you to anything. Any charge for care is between you and the provider, and they will quote it before any visit.

How many providers will contact me?

At most one. We route a request to a single matched provider rather than selling it to several, so you should not receive a wave of calls.

What if I change my mind?

Tell the provider you are no longer interested, and contact us if you would like our copy of your request deleted. Consent is not a condition of purchasing any service and you can withdraw it.
